Boas.
Por aqui é bastante comum ter ataques de bots em fóruns SMF, mesmo com o Captcha activo.
Nos fóruns mais movimentados, acontece menos, no entanto até em fóruns em desenvolvimento me acontece este problema.

Que solução recomendam?

Os bots facilmente confirmam esses links de email. O melhor é colocares um captcha diferente.

Nunca usei smf por isso não te consigo dizer nenhum em especifico, mas procura por um custom captcha onde és tu que colocas as perguntas para eles responderem. Assim, vais reduzir imenso o spam, se nao mesmo acabar com ele.
