Instalei finalmente o opensef e os endereços funcionam bem mas em algumas paginas aparece o seguinte erro:

E que tal desinstalares e instalares outro tipo sh404sef, é que esse já foi descontinuado, se calhar esses erros estãoa surgir por alguma incompatibilidade com a última versão do Joomla que mudou algumas coisas na base de dados.
Com o sh404sef nem precisas de .htaccess para ele funcionar.
